Do you accept insurance?
I am a cash-based service and do not directly accept insurance as means of payment with the exception of Medicare. However, upon request I am able to provide an itemized bill that you can personally submit to your insurance company for reimbursement. I am able to accept HSA and FSA for any physical therapy service. A 10% discount is provided to those paying cash for physical therapy services.
Do you take clients that have Medicare?
Yes. Medicare Part B is the only insurance I accept. Medicare Advantage plans are not accepted at this time.
Do I need an order from a physician?
That depends. Due to direct access laws in Illinois, I am able to perform a functional assessment without a physician's order. If it is deemed that physical therapy is the appropriate service, then most clients will then need to obtain a physician's order to continue with treatment. If an order is needed, I will provide direction for the additional steps required.
How far do you travel?
I provide services to the following cities/towns: Peoria, Bartonville, Deer Creek, Dunlap, East Peoria, Eureka, Germantown Hills, Goodfield, Mackinaw, Metamora, Morton, Pekin, Roanoke, Tremont, and Washington. I am available to travel to other areas at an additional cost.
On Wednesdays travel to Springfield area is available without travel fees.
Do I have to be home bound?
No. Lion Concierge Physical Therapy Services is coded as an outpatient physical therapy clinic. Though I provide services in my clients' homes, it is separate from traditional home health and my clients are able to receive services without the home bound limitation
